VideosHelpDutiesThe Licensed Practical Nurse exhibits independent technical judgment, critical thinking skills, and the use of independent judgment to carry out complex patient-assigned duties, which include prioritization and coordination of patients seen in the Telehealth clinic with minimal supervision.
The LPN proactively assists the Physician in addressing patient's need for medication/treatments, accurately administering prescribed medications according to established policies/procedures.
Performs direct patient care activities, as appropriate to assignment, (includes, but is not limited to):
Scheduling Telehealth visits for both the provider side appointments and the patient side appointments Use data analysis, and performance/qualify improvement roles for the Telehealth program and clinics Assesses, records, and reports vital signs, neurological signs, weight, and blood glucose level Appropriately triages and prioritizes patient needs as routine, urgent, or emergent in the clinic setting and modifies patient care/treatment as needed Checks patients into the clinic, introduce the patient to the Telehealth modality and to the Telehealth provider.
Possesses comprehensive knowledge of VA and National Telehealth protocols and VHA/ National Telehealth directives related to clinic management, documentation, and computer and Telehealth equipment technology Serves as a resource for complex/specialized procedures (i.
e.
non-complex suture removal, non-complex wound care, blood draws, and clinic specific procedures, such as ear irrigation).
Performs venipuncture for blood draws, prepares specimens for transport and processing (centrifuge and serum separation), transcribes orders, accessions labels, provides patient education about lab tests, etc Assists with diagnostic procedures that include preparing the patient, setting up sterile field and instrumentation, and monitoring the patient's condition Demonstrates the knowledge and skill to interpret common test results and other medical data.
Provides education to patients/families about preparation for procedures and/or diagnostic tests.
Refer the patient for specialized Telehealth clinics to assist with disease prevention and health promotion.
Demonstrates knowledge and skill to incorporate patient education into care and modifies as necessary to meet needs of the patient using TEACH for Success and other health coaching strategies Collaborates with the Telehealth provider and other interdisciplinary services in the development, implementation, and evaluation of patient care Serves in a leadership role within the Telehealth team to promote holistic care and optimize clinic efficiency.
Assists other staff to improve and advance their practical nursing practice as appropriate.
Serves as a resource person for frequently used Telehealth equipment in the clinic setting and utilizes appropriate protocol for equipment repair and maintenance.
Functions as a role model for co-workers in guiding clinical practice Active participant in quality improvement projects/hospital committees to promote evidence-based practice and performance improvement in the Telehealth setting Screening, Pain Screening, Frail/Elderly Screening, etc.
Develops and implements action plans, including educating nursing staff to address identified deficiencies Leads Telehealth huddles effectively and serves as a resource for new clinical and administrative staff members re:
Telehealth principles Demonstrates advanced knowledge and skill in preparation, administration, and documentation of commonly prescribed oral, topical, subcutaneous, and intramuscular medications permitted by facility policies and procedures and within the LPN Expanded Scope of Practice.
Examples include administering immunizations, testosterone, influenza vaccinations, herpes zoster, pneumococcal, and Vitamin B12 injections.
Ensures all follow-up results notes, future consults, peer reviews and completed using telemedicine protocols.
Delivers virtual and digital technologies which help veterans communicate synchronously and asynchronously with their health care teams and coordinate, track and manager their health care.
These technologies may include , but are not limited to, Clinical Video Telehealth (CVT), Store and Forward Telehealth (SFT), Video Conferencing, VA Video Connect (VVC), VA Mobile, My HealthVet (MHV), Home Telehealth (HT), and other telehealth applications.

Education:
Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant.

License:
Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.
e.
, Puerto Rico) of the United States, or District of Columbia.
An LPN/LVN who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16 of this part.
English Language Proficiency.
Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.

Requires at least 1 year of additional qualifying experience at the GS-5 level or equivalent and fully meeting all performance requirements for the GS-5 LPN.

Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations.
Knowledge and ability to appropriately carry out assigned patient care based on the patient' conditions.
Ability to observe, identify, and respond to the patient's needs for care, including medication, equipment-assisted care, and patient/family education.
Prepare and administer prescribed medication (oral, topical, subcutaneous, intramuscular) and performs treatments according to established policies/procedures.
Observes for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or MD/DO.
Knowledge and ability to recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO,and initiate appropriate emergency interventions as directed.
Knowledge and understanding of human behavior, patient motivations and reactions to situations, and ability to appropriately utilize this knowledge in working effectively with patients, family members, and other staff.
Establish constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication, and socialization skills.
Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure.
Actively seek out educational opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice.
